Partial ileal bypass surgery is a surgical procedure which involves shortening the ileum to shorten the total small intestinal length. [1] First introduced in 1962 by Professor Henry Buchwald of the University of Minnesota, [2] the procedure is used to treat a number of hyperlipidemias including familial hypercholesterolemia.

Ileotransverse colostomy with mucous fistula is utilized when the disease process involves the major portion of the terminal ileum plus the ascending colon. The transverse colon may be the ideal site for a small bowel bypass, since it has had little if any ionizing irradiation.

In those cases where the small bowel is involved with obstruction and/or fistula formation following total pelvic irradiation and/or advanced malignant disease is impacted in the true pelvis, a bypass with mucous fistula rather than a bowel resection is the operation of choice.

Partial ileal bypass (PIB) is a safe, effective, and lasting therapy for the reduction of lipids and lipoproteins in patients with hyperlipidemia. Following PIB, circulating plasma and low-density lipoprotein (LDL) cholesterol fall markedly, while high-density lipoprotein (HDL) cholesterol rises.
